New York ACA Benefits and Coverage: Your Complete 2026 Guide

Learn about New York ACA benefits and coverage options including Essential Plan, Medicaid, marketplace subsidies, income limits, and how to apply through NY State of Health.

New York ACA Benefits and Coverage: Your Complete 2026 Guide

New York residents have access to some of the most comprehensive health insurance options in the country, thanks to the Affordable Care Act (ACA) and the state's own programs. Whether you are uninsured, switching jobs, or simply looking for more affordable coverage, understanding your New York ACA benefits and coverage options can save you thousands of dollars each year.

This guide breaks down everything you need to know about health coverage in New York, including income limits, what is covered, how to apply, and programs unique to the Empire State.

How the ACA Works in New York

New York operates its own state-based health insurance marketplace called NY State of Health (nystateofhealth.ny.gov). Unlike states that use the federal HealthCare.gov platform, New York manages enrollment, plan selection, and eligibility determinations through its own system.

Through NY State of Health, you can access:

  • Qualified Health Plans (QHPs) with premium tax credits for individuals and families above 250% of the Federal Poverty Level (FPL)
  • The Essential Plan, a unique New York program offering $0 premium coverage for those earning 138% to 250% FPL
  • Medicaid, covering individuals and families up to 138% FPL
  • Child Health Plus, providing coverage for children up to 400% FPL

New York expanded Medicaid under the ACA, which means there is no coverage gap in the state. Every income level has a corresponding health coverage option.

The 10 Essential Health Benefits Under the ACA

All ACA marketplace plans in New York must cover 10 categories of essential health benefits. These are:

  1. Ambulatory patient services (outpatient care you receive without being admitted to a hospital)
  2. Emergency services (emergency room visits)
  3. Hospitalization (inpatient care including surgery and overnight stays)
  4. Maternity and newborn care (prenatal visits, labor and delivery, postnatal care)
  5. Mental health and substance use disorder services (therapy, counseling, inpatient treatment)
  6. Prescription drugs (generic and brand name medications)
  7. Rehabilitative and habilitative services (physical therapy, occupational therapy, speech therapy)
  8. Laboratory services (blood tests, imaging, diagnostics)
  9. Preventive and wellness services (annual checkups, immunizations, screenings)
  10. Pediatric services (dental and vision care for children)

New York goes beyond federal minimums in several areas. The state mandates additional coverage for services like infertility treatment, autism spectrum disorder therapies, and chiropractic care. All marketplace plans must also cover contraception with no cost sharing.

ACA Marketplace Plans (Qualified Health Plans)

If your income is above 250% FPL, you can purchase a Qualified Health Plan through NY State of Health. These plans come in four metal tiers:

  • Bronze: Lowest premiums, highest out of pocket costs. Good if you rarely need care.
  • Silver: Moderate premiums and cost sharing. Best value if you qualify for cost sharing reductions.
  • Gold: Higher premiums, lower out of pocket costs. Good for frequent medical visits.
  • Platinum: Highest premiums, lowest out of pocket costs. Best for those who use healthcare regularly.

Premium Tax Credits reduce your monthly premium based on your income. The lower your income, the larger the subsidy. With enhanced ACA subsidies (currently in effect through 2025), no one pays more than 8.5% of household income toward the benchmark Silver plan premium.

Child Health Plus

For families with children, Child Health Plus covers kids under 19 with family income up to 400% FPL. Premiums range from free (for incomes up to 222% FPL) to $60 per child per month at the highest income tier, with a family cap of $180 per month.

Child Health Plus enrollment is open year round.
