Iowa ACA Eligibility 2026: Income Limits, Subsidies, and How to Enroll

Find out if you qualify for ACA marketplace health insurance in Iowa for 2026. Learn about income limits, premium subsidies, Medicaid eligibility, and step-by-step enrollment instructions.

If you live in Iowa and need health insurance, understanding your ACA eligibility for 2026 is the first step toward getting covered. Iowa uses the federal marketplace at Healthcare.gov for ACA plan enrollment, and more than 123,000 Iowans selected a marketplace plan for 2026 coverage. However, significant changes to premium subsidies this year mean your costs could look very different from previous years.

This guide breaks down exactly who qualifies for ACA coverage in Iowa, what income limits apply in 2026, how subsidies have changed, and how to enroll or find alternatives like Medicaid.

How the ACA Marketplace Works in Iowa

Iowa does not run its own state health insurance exchange. Instead, Iowa residents shop for plans, compare coverage, and enroll through the federally run marketplace at Healthcare.gov. Six private insurance carriers offered health plans on Iowa's marketplace for 2026.

Iowa Marketplace Quick Facts for 2026

  • Exchange type: Federally facilitated (Healthcare.gov)
  • 2026 enrollment: Approximately 123,304 individuals selected a plan
  • Number of carriers: 6
  • Year over year change: Down about 9.9% from 2025
  • Medicaid expansion: Yes, Iowa has expanded Medicaid
  • Marketplace phone: 1-800-318-2596

The enrollment decline from 2025 is largely tied to changes in federal subsidy rules, which we cover in detail below.

Who Is Eligible for ACA Marketplace Coverage in Iowa?

To buy a health insurance plan through the Iowa marketplace, you must meet these basic requirements:

  • Residency: You must live in Iowa
  • Citizenship: You must be a U.S. citizen, U.S. national, or lawfully present in the country
  • Not incarcerated: You cannot be currently incarcerated
  • Not on Medicare: You cannot be enrolled in Medicare

Meeting these criteria makes you eligible to purchase a marketplace plan. However, qualifying for financial help (premium subsidies and cost-sharing reductions) depends on your income and whether you have access to other affordable coverage.

To receive premium tax credits, you must also:

  • Not have access to affordable employer-sponsored health insurance
  • Not qualify for Medicaid or CHIP
  • Not be eligible for premium-free Medicare Part A
  • File a joint tax return if married
  • Not be claimed as a tax dependent by someone else

2026 Income Limits for ACA Subsidies in Iowa

Your household income determines whether you qualify for premium tax credits that lower your monthly insurance costs. For 2026, subsidies are available to individuals and families with income between 100% and 400% of the Federal Poverty Level (FPL).

One major change for 2026: the enhanced subsidies that were in place from 2021 through 2025 under the American Rescue Plan (ARP) and Inflation Reduction Act (IRA) have expired. This means the "subsidy cliff" has returned. If your household income exceeds 400% of FPL, you no longer qualify for any premium assistance, regardless of how much your premiums cost.

2026 ACA Subsidy Income Limits (100% to 400% FPL)

Household Size100% FPL (Minimum)400% FPL (Maximum for Subsidies)
1$15,960$63,840
2$21,640$86,560
3$27,320$109,280
4$33,000$132,000
5$38,680$154,720
6$44,360$177,440

If your income falls within this range, you likely qualify for premium tax credits that reduce your monthly premiums on a marketplace plan. The exact amount of your subsidy depends on your income, age, and the cost of the benchmark Silver plan in your area.

Cost-Sharing Reductions

If your household income is at or below 250% of FPL, you may also qualify for cost-sharing reductions (CSRs). These lower your deductibles, copays, and out-of-pocket maximums. To receive CSR benefits, you must enroll in a Silver-level plan.

Household Size250% FPL (CSR Threshold)
1$39,900
2$54,100
3$68,300
4$82,500

What Changed with ACA Subsidies in 2026

The expiration of the ARP/IRA enhanced subsidies at the end of 2025 brought two significant changes:

1. The subsidy cliff is back. Before 2021, anyone earning above 400% FPL received zero premium assistance. The ARP removed that cap so higher-income households could still get help if premiums exceeded 8.5% of their income. That expansion is now gone. If you earn $63,841 as an individual, you get no subsidy at all.

2. Subsidies are smaller for everyone else. Even if your income falls between 100% and 400% FPL, your premium tax credit will be smaller than it was in 2024 or 2025. The ARP had reduced the percentage of income people were expected to contribute toward premiums. Those lower contribution percentages have reverted to the original ACA formula.

For context, during the 2025 open enrollment period, 88% of Iowa marketplace enrollees qualified for premium subsidies that saved them roughly $470 per month on average. Average after-subsidy premiums for those receiving assistance were about $99 per month. Costs in 2026 will generally be higher for most enrollees.

Iowa Medicaid Eligibility: An Alternative to Marketplace Plans

Iowa expanded Medicaid under the ACA, which means low-income adults can qualify for Medicaid coverage even if they do not have children. If your income is at or below 138% of FPL, you may qualify for Iowa Medicaid instead of a marketplace plan.

2026 Medicaid Income Limits in Iowa (138% FPL)

Household SizeAnnual Income LimitMonthly Income Limit
1$22,025$1,835
2$29,863$2,489
3$37,702$3,142
4$45,540$3,795
5$53,378$4,448
6$61,217$5,101

Iowa Medicaid covers a broad range of services, often with little to no out-of-pocket costs. If your income is near the Medicaid threshold, applying through the Iowa HHS Benefits Portal or Healthcare.gov will automatically check whether you qualify for Medicaid or marketplace subsidies.

Additional Iowa Medicaid Categories

Beyond the income-based expansion for adults aged 19 to 64, Iowa Medicaid also covers:

  • Children under 21 (income limits vary, with CHIP covering children up to 302% FPL)
  • Pregnant women (up to approximately 215% FPL)
  • Parents and caretaker relatives
  • Elderly individuals aged 65 and older
  • Individuals with disabilities meeting Social Security standards
  • Former foster care youth

How to Apply for ACA Coverage in Iowa: Step by Step

Step 1: Gather Your Information

Before you start, have the following ready for each household member:

  • Social Security numbers
  • Date of birth
  • Immigration documents (if applicable)
  • Employer and income information (pay stubs, W-2s, or tax returns)
  • Current health insurance policy numbers (if you have existing coverage)

Step 2: Create or Log Into Your Healthcare.gov Account

Visit Healthcare.gov and create an account or sign in to an existing one. You will need a valid email address and will be asked to create a username and password.

Step 3: Complete Your Application

Fill out the marketplace application with information about your household, income, and any current coverage. The system will determine whether you qualify for:

  • Premium tax credits (subsidies)

Step 4: Compare Plans and Choose Coverage

Enrollment Periods and Special Enrollment

The open enrollment period for 2026 coverage ended on January 15, 2026. Starting with the fall 2026 enrollment period (for 2027 coverage), the deadline will move earlier to December 15 due to a new federal rule.

Outside of open enrollment, you can only sign up or change plans if you experience a qualifying life event, such as:

  • Losing existing health coverage (from a job, Medicaid, or another source)
  • Getting married or divorced
  • Having or adopting a child
  • Moving to a new area with different plan options
  • Turning 26 and losing coverage under a parent's plan
  • Changes in household income that affect Medicaid eligibility

Native Americans and Alaska Natives can enroll in marketplace plans at any time during the year without a qualifying life event.

If you have experienced a qualifying life event, you typically have 60 days to enroll in a new plan through Healthcare.gov.

Frequently Asked Questions

Can I still get ACA subsidies in Iowa if I earn over 400% FPL?

No. Starting in 2026, the subsidy cliff has returned. Households with income above 400% of the Federal Poverty Level ($63,840 for an individual, $132,000 for a family of four) no longer qualify for premium tax credits. This is a change from 2021 through 2025, when enhanced subsidies removed the income cap.

What if my income is too low for ACA subsidies?

If your income falls below 100% of FPL, you generally will not qualify for marketplace subsidies. However, because Iowa expanded Medicaid, most low-income adults qualify for Medicaid at incomes up to 138% FPL. Apply through Healthcare.gov or the Iowa HHS Benefits Portal to see which program covers you.

I missed open enrollment. Can I still get coverage?

Open enrollment for 2026 plans ended January 15, 2026. You can still enroll if you have a qualifying life event (job loss, marriage, birth of a child, loss of other coverage, etc.). You generally have 60 days from the event to apply. Visit Healthcare.gov to start a special enrollment application.

How do I know if I should choose Medicaid or a marketplace plan?

When you apply on Healthcare.gov, the system automatically checks whether you qualify for Medicaid based on your income. If you are eligible for Medicaid, you will be directed to that program. Medicaid typically has lower or no out-of-pocket costs compared to marketplace plans.

Are there free or low-cost plans available?

With subsidies, some Iowa residents may find Bronze or Silver plans with very low monthly premiums. However, with reduced subsidy amounts in 2026, $0 premium plans are less common than they were in recent years. Use Healthcare.gov to see your specific options and estimated costs.

What documents do I need to apply?

You will need Social Security numbers for each household member applying, proof of income (pay stubs, W-2 forms, or a recent tax return), and any immigration documentation if applicable. Having this information ready before you start will speed up the process.

Can I apply for both Medicaid and ACA at the same time?

Yes. A single application on Healthcare.gov or through the Iowa HHS Benefits Portal will check your eligibility for both Medicaid and ACA subsidies simultaneously. You do not need to submit separate applications.
